Change of Address Guidelines

  • Parents of students currently enrolled in the Massapequa School District must notify the District Central Registrar and reprove residency when there is a change in address. This must be done whether or not transportation services are provided or there is a change in school placement.     Step 1: Complete the online change of address form

    Step 2: Submit all mandatory documentation accordingly to the District Central Registrar

    HOMEOWNERS Documents to be submitted

    • Affidavit A - must be signed and notarized. Click here for a copy of Affidavit A
    • Copy of deed or current property tax bill/receipt
    • Two billing statements (one of the following choices):
      • Current Mortgage statement and a utility bill (Water, gas, electric, or cable - wireless telephone bills are not accepted
      • Two current utility bills (consecutive months - Water, gas, electric, or cable - wireless telephone bills are not accepted)
    • Copy of parent(s) Photo ID (Driver's License, Passport, or State issued photo identification card)

          NON-HOMEOWNERS - Documents to be submitted

    By Homeowner

    • Affidavit B - must be signed and notarized by Homeowner  Click here for a copy of Affidavit B
    • Copy of house/apartment lease or rental agreement signed by the owner/landlord and renter
    • Copy of owner/landlord’s deed or current property tax bill
    • Copy of one utility bill
    • Copy of landlord’s valid driver’s license, passport or State issued identification card and contact phone number

    By Renter/Lessee

    • Affidavit C - must be signed by renter/parent and notarized.  Click here for a copy of Affidavit C
    • Two (2) forms of residency which can include utility bills, vehicle insurance ID cards, tax returns, or payroll notifications.
    • Renter's current bank statement addressed to rental property address
    • Copy of parent(s) Photo ID (Driver's License, Passport, or State issued photo identification card)

     

    IMPORTANT: THE CHANGE OF ADDRESS PROCESS IS NOT COMPLETE UNTIL THE CHANGE OF ADDRESS FORM AND ALL DOCUMENTATION IS SUBMITTED AND APPROVED BY THE DISTRICT CENTRAL REGISTRAR.
