Change of Address Guidelines

  • Parents of students currently enrolled in the Massapequa School District must notify the District Central Registrar and reprove residency when there is a change in address. This must be done whether or not transportation services are provided or there is a change in school placement.     Complete the MSD Change of Address Form, located on the Parent Portal via the Form icon.

    Please note:  In order to submit your child's MSD Change of Address form, you will be required to upload all of the mandatory documentation noted within the application process.  Click here for a list of all mandatory documentation.

    IMPORTANT: THE CHANGE OF ADDRESS PROCESS IS NOT COMPLETE UNTIL THE MSD CHANGE OF ADDRESS FORM AND ALL DOCUMENTATION IS SUBMITTED AND APPROVED BY THE DISTRICT CENTRAL REGISTRAR.
